Privacy Policy

This Privacy Policy discloses the privacy practices for Corporate Health Systems, Inc. (CHS). This Privacy Policy applies to all information we collect from you through corphealthsys.com and other websites operated by us from which you are accessing this Privacy Policy. We will refer to these websites as “Services” throughout this Privacy Policy. Consent to Use CHS’s Services By providing personal information to us through CHS’s Services you are consenting to the collection, use, and disclosure of your personal information in accordance with this Privacy Policy. If you do not agree with the practices described in this Privacy Policy, please do not provide personal information through CHS’s Services.

Information Collection and Use CHS is the sole owner of the information collected through its Services. We collect personal information that you voluntarily provide through our Services. Such information may include:

  • Name, address and birthday;
  • Other contact information such as email address;
  • Social security or similar national ID number;
  • Information about individual accounts and account histories; and

Information obtained from independent third parties.

We collect and use personal information you provide when you visit or use our Services to fulfill the purposes for which you provided the information and to enhance your experience with us. These uses include:

We do not rent, sell, or otherwise share information about users to others in ways different from what is disclosed in this Privacy Policy. We will not disclose nonpublic information about our users without their express permission, except as permitted or required by law. We also ensure reasonable measures to protect the privacy of our users’ information through physical, electronic, and procedural safeguards.

Session Cookies

CHS uses session cookies to keep a person logged into their account while using CHS’s Services. A session cookie is a line of text that is stored temporarily in your web browser’s memory cache and will expire when the browser is closed. When a user ends the session the session cookie is deleted and is not retained on the member’s computer. Users have the option of disabling or not accepting cookies by changing the preferences on their browsers.* However, if a user opts to disable cookies, they will not be able to access all information and resources on CHS’s Services. No personally identifiable information (e-mail address, name, etc.) is collected with the cookies that we set. Accepting cookies while on the CHS’s Services will not put the member at risk for marketing to other websites.

Web Statistics

An “IP Address” (which is defined as a set of numbers that identifies each computer using the Internet Protocol to communicate over a network) may be identified and logged automatically in our server log files whenever a user access the Services, along with the time of the visit and the page(s) that were visited. We use IP Addresses to analyze trends, administer the site, track user movement, and gather broad demographic information for aggregate use for reporting and sponsorship purposes. IP addresses are not linked to personally identifiable information.


We do not share aggregated demographic information with our partners. We will share aggregated utilization information and system statistics with our clients for utilization reporting purposes. This information is not linked to any personal information that can identify any individual person.


CHS’s Services contain links to other websites operated by third parties. Please be aware that CHS does not claim any responsibility for the privacy practices of such other sites. We encourage our members to be aware when they leave our site and to read the privacy statements of each and every website that collects personally identifiable information. This privacy statement applies solely to information collected by CHS’s Services.


CHS takes every precaution to protect our users’ information, and we have security measures in place to protect the loss, misuse, and alteration of the information under our control. The security measures in place meet the requirements of the Health Insurance Portability and Accountability Act (HIPAA).


Occasionally, CHS requests information from users via Web surveys. Participation in almost all surveys or contests is completely voluntary and the user therefore has a choice whether or not to disclose this information. Information requested may include demographic data (such as zip code and age), but nothing that can identify any individual person will be collected. Survey results will be used for purposes of monitoring or improving the use and satisfaction of this site, and internal program and service development.


We may also send the user site and service announcement updates. Users are not able to unsubscribe from service announcements that contain important information about the service. We communicate with users to provide requested services and to discuss issues relating to their accounts via e-mail or phone.

Correction/Updating Personal Information

If a user’s personally identifiable information changes (such as zip code or e-mail address), or if a user no longer desires our service, we will endeavor to provide a way to correct, update, or remove that the personal data that was provided to us. Users can use the "Settings" link or they can e-mail our Help Desk to change, correct, or remove information.


Our users are given the opportunity to "opt-out" of newsletters and general communications including e-mail messages. To unsubscribe from mailings, please see the unsubscribe link at the bottom of all e-mail messages.

Notification of Changes

CHS reserves the right to amend this Privacy Policy at any time. If we decide to change our privacy policy, we will post those changes on our websites so our members are always aware of what information we collect, how we use it, and under what circumstances, if any, we disclose it. If at any point we decide to use personally identifiable information in a manner different from that stated at the time it was collected, we will notify members by e-mail. Users will have a choice as to whether or not we use their information in this different manner. We will use information in accordance with the privacy policy under which the information was collected.

Our Online Privacy Notice for Children

Our services are not directed or intended for children. CHS does not knowingly collect or post information from children under the age of 13 in compliance with laws protecting the privacy of children.
